I am aware that my child will be borrowing a costume from the studio for our Holiday Showcase Performance. The costume they will be borrowing is our Santa Dress Costume. By typing my name below & signing this agreement, I am aware that the costume must be returned immediately after the performance. The cost of this costume is $79. If the costume is not returned, or not returned in it's original condition, this amount will be automatically charged to the credit card on my student's file.
